- Coordinate patient care: The Rehab Liaison will be responsible for coordinating the care of patients, working closely with physicians, therapists, and other healthcare professionals to ensure the best possible outcomes for patients.
- Conduct patient assessments: The Rehab Liaison will conduct thorough assessments of patients' medical history, current condition, and rehabilitation needs in order to develop individualized treatment plans.
- Educate patients and families: The Rehab Liaison will provide education to patients and their families regarding the rehabilitation process, treatment options, and expectations for recovery.
- Facilitate communication: The Rehab Liaison will act as a liaison between patients, families, and healthcare providers to ensure open and effective communication throughout the rehabilitation process.
- Monitor patient progress: The Rehab Liaison will closely monitor patient progress and make necessary adjustments to treatment plans as needed to ensure the best possible outcomes.
- Collaborate with interdisciplinary team: The Rehab Liaison will work closely with other members of the healthcare team, including physicians, therapists, nurses, and social workers, to provide comprehensive and coordinated care for patients.
- Maintain accurate records: The Rehab Liaison will be responsible for maintaining accurate and up-to-date patient records, including assessments, treatment plans, progress notes, and discharge plans.
- Adhere to policies and procedures: The Rehab Liaison will adhere to all policies and procedures of Deaconess Health System to ensure the safety and well-being of patients and staff.
- Stay current on industry trends: The Rehab Liaison will stay informed on the latest advancements and best practices in rehabilitation to continuously improve the quality of care provided to patients.
- Uphold patient confidentiality: The Rehab Liaison will maintain strict confidentiality regarding patient information in accordance with HIPAA regulations and Deaconess Health System policies.
Bachelor's Degree In A Related Field Such As Healthcare Administration, Nursing, Or Social Work.
Minimum Of 2 Years Experience In A Healthcare Or Rehabilitation Setting, Preferably In A Liaison Or Coordination Role.
Strong Communication And Interpersonal Skills, With The Ability To Collaborate And Build Relationships With Healthcare Providers, Patients, And Their Families.
Knowledge Of Insurance And Reimbursement Processes, As Well As An Understanding Of Regulatory Requirements For Rehabilitation Services.
Ability To Work Independently And Manage Multiple Tasks And Projects, With Excellent Organizational And Time Management Skills.
