Patient Placement

In this role, you will be responsible for the placement of patients within the hospital, ensuring that each patient is placed in the most appropriate setting to meet their medical and financial needs. This position requires a detail-oriented, organized individual with excellent communication skills and the ability to work in a fast-paced environment. The ideal candidate must possess a minimum of three years of experience in patient placement and/or utilization review. Responsibilities:

1. Facilitate the placement of patients into appropriate care settings, considering medical and financial needs.
2. Communicate regularly with patients and families to ensure they understand their placement options.
3. Monitor patients’ progress, adjusting placement as needed.
4. Ensure compliance with all relevant regulations and policies.
5. Serve as a resource for other members of the healthcare team regarding patient placement and utilization review.
6. Develop and maintain relationships with external vendors and partners.
7. Monitor utilization and performance data to identify opportunities for improvement.
8. Develop and implement strategies to improve patient placement and utilization review processes.
9. Create and maintain detailed records of all patient placements and utilization reviews.
10. Participate in educational programs and professional development opportunities to stay up-to-date on best practices.
