  1. Coordinate and facilitate the transition of care for patients between different healthcare settings.

  4. Communicate effectively with all stakeholders involved in the care transition process.
  5. Monitor and track patient progress and outcomes to identify areas for improvement.
  6. Ensure compliance with all regulatory requirements and company policies.
  7. Educate patients and caregivers on medication management, appointments, and other important healthcare information.
  8. Identify and address any barriers to successful care transitions, such as transportation or financial concerns.
  9. Maintain accurate and detailed documentation of all care transition activities.
  10. Participate in ongoing training and education to stay updated on healthcare industry trends and best practices.
  11. Contribute to the development and implementation of new processes and procedures to improve care transition outcomes.
  12. Adhere to the company's mission and values, and represent the organization in a professional manner at all times.
