- Conduct research and analyze data to identify key social determinants of health in various communities.
- Collaborate with cross-functional teams to integrate social determinants of health into existing programs and initiatives.
- Build relationships with community partners and stakeholders to coordinate efforts and maximize impact.
- Develop and maintain a deep understanding of the social determinants of health landscape, including current trends and best practices.
- Advocate for the importance of addressing social determinants of health within the healthcare industry.
- Monitor and evaluate the effectiveness of implemented strategies and recommend adjustments as needed.
- Communicate and present findings, recommendations, and progress to internal and external stakeholders.

- Train and educate team members on the importance of social determinants of health and how to incorporate it into their work.
- Represent CVS Health at conferences, events, and meetings related to social determinants of health.
Knowledge And Understanding Of Social Determinants Of Health (Sdoh): A Successful Candidate For This Role Should Have A Deep Understanding Of Sdoh And How They Impact Health Outcomes And Health Disparities. This Includes Knowledge Of Key Social And Environmental Factors Such As Education, Employment, Housing, And Access To Healthcare.
Experience In Community Engagement And Partnership Building: As A Community Strategist, The Individual Will Be Responsible For Working With Community Organizations, Local Government, And Other Stakeholders To Develop And Implement Initiatives That Address Sdoh. Therefore, Experience In Community Engagement And Partnership Building Is Essential For Success In This Role.
Strong Analytical And Problem-Solving Skills: The Ability To Collect, Analyze, And Interpret Data Related To Sdoh Is Crucial For A Community Strategist. This Includes Identifying Trends And Patterns In Data To Inform Strategic Decisions And Problem-Solving Skills To Develop Effective Solutions.
