Job Description

Are you passionate about advocating for underserved communities and addressing health disparities? CVS Health is seeking a Social Determinants of Health Community Strategist to join our team and drive our efforts in promoting health equity. In this role, you will lead our community outreach and partnership initiatives, working closely with local organizations and stakeholders to address social determinants of health and improve health outcomes for all. We are looking for a highly motivated and experienced individual who shares our commitment to making a positive impact on the health of our communities.   1. Develop and implement community outreach and partnership initiatives to address social determinants of health.
  2. Collaborate with local organizations and stakeholders to identify health disparities and develop targeted strategies to promote health equity.
  3. Conduct research and analysis to identify key issues and trends related to social determinants of health.
  4. Develop and maintain relationships with community partners to ensure effective collaboration and maximize impact.
  5. Create and deliver presentations and trainings to educate stakeholders on social determinants of health and strategies for addressing them.
  6. Monitor and evaluate the effectiveness of community initiatives and make recommendations for improvement.
  7. Stay current on national and local policies and initiatives related to health equity and social determinants of health.
  8. Represent CVS Health at community events and meetings to raise awareness and advocate for health equity.
  9. Develop and manage budgets for community outreach and partnership initiatives.
  10. Collaborate with internal teams to integrate social determinants of health strategies into company-wide initiatives and programs.
  11. Foster a culture of diversity, inclusion, and cultural competency within the organization.
  12. Serve as a subject matter expert on social determinants of health for the organization and provide guidance and support to colleagues.
  13. Act as a liaison between CVS Health and community organizations, communicating our goals and initiatives and identifying opportunities for collaboration.
  14. Stay informed on best practices and emerging trends in addressing social determinants of health to continuously improve our approach.
  15. Serve as a champion for health equity and advocate for underserved communities within the organization and externally.

Job Qualifications
  • Knowledge Of Social Determinants Of Health (Sdoh): A Strong Understanding Of The Various Sdoh Factors And Their Impact On Health Outcomes Is Essential For A Community Strategist At Cvs Health. This Includes Knowledge Of Topics Such As Poverty, Education, Housing, Food Insecurity, And Access To Healthcare.

  • Experience In Community Outreach: The Community Strategist Will Be Responsible For Building Relationships And Partnerships With Community Organizations And Stakeholders. Previous Experience In Community Outreach And Engagement Is Important In Order To Effectively Identify And Address Local Health Needs.

  • Analytical Skills: The Ability To Collect, Analyze, And Interpret Data Related To Sdoh Is Crucial For This Role. The Community Strategist Will Need To Use Data To Identify Health Disparities And Develop Targeted Interventions To Address Them.

  • Communication And Collaboration Skills: Strong Communication And Collaboration Skills Are Essential For A Community Strategist. This Role Involves Working With A Variety Of Stakeholders, Including Community Members, Healthcare Providers, And Government Agencies. The Ability To Effectively Communicate And Collaborate With These Groups Is Key To Success In This Role.

  • Cultural Competency: Given The Diverse Communities That Cvs Health Serves, It Is Important For The Community Strategist To Have A Strong Understanding Of Cultural Competency. This Includes Being Able To Work With People From Different Backgrounds And Understanding How Cultural Factors May Impact Health Outcomes.
