Job Description

CVS Health is seeking an experienced and organized Patient Care Coordinator to join our team. This is an exciting opportunity for someone looking to make an impact in the healthcare industry. Responsibilities:

  1. Coordinate patient care services, including scheduling of appointments and ensuring timely delivery of services.
  2. Provide excellent customer service to patients, staff, and vendors.

  4. Think critically and problem solve in order to provide the best patient care.
  5. Develop and maintain relationships with patients, staff, and vendors.
  6. Communicate effectively with all stakeholders involved in patient care.
  7. Work independently and collaboratively as part of a healthcare team.
  8. Utilize strong organizational skills to manage patient care services.
  9. Monitor and assess patient care services to ensure quality.
  10. Document patient care activities and outcomes.
