Responsibilities:

1. Provide direct nursing care and support to CVS Health members.
2. Develop and maintain strong relationships with CVS Health members to ensure their health and well-being.
3. Assist in improving the overall quality of care by providing evidence-based nursing services.
4. Maintain accurate and timely documentation of patient care.
5. Monitor patient progress, assess medical needs, and identify any risks or issues.
6. Work collaboratively with other CVS Health team members to ensure the delivery of high-quality patient-centered care.
7. Educate patients and families on disease processes, treatment options, and self-care strategies.
8. Participate in patient care conferences and provide input and recommendations regarding patient care.
9. Participate in quality improvement initiatives and clinical research activities.
