Job Description

As a Clinical Case Manager, you will play a crucial role in coordinating and managing the treatment and care of our behavioral health patients.   1. Provide comprehensive case management services to patients with behavioral health issues.
  2. Conduct initial assessments and develop individualized treatment plans in collaboration with patients and their families.
  3. Coordinate and monitor the implementation of treatment plans, ensuring that all services are delivered in a timely and effective manner.
  4. Collaborate with healthcare providers, including psychiatrists, therapists, and social workers, to ensure coordinated and integrated care for patients.
  5. Maintain accurate and up-to-date documentation of patient progress, treatment plans, and any changes in their condition.
  6. Monitor and track patient outcomes and make adjustments to treatment plans as needed.
  7. Advocate for patients and their families to ensure access to necessary resources and services.
  8. Provide crisis intervention and support to patients and their families as needed.
  9. Facilitate communication and collaboration between patients, families, and healthcare providers to promote a holistic approach to care.
  10. Stay up-to-date with industry trends and best practices in behavioral health to continuously improve service delivery and patient outcomes.
  11. Maintain confidentiality and adhere to all regulatory and ethical standards in the handling of patient information.
  12. Participate in team meetings and case conferences to discuss patient progress and develop strategies for improved care.
  13. Assist in the development and implementation of policies and procedures related to behavioral health services.
  14. Act as a liaison between CVS Health and community resources to ensure a comprehensive network of support for patients.
  15. Demonstrate a commitment to the mission and values of CVS Health, promoting a positive and supportive work environment.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Psychology, Or Related Field

  • Minimum Of 2 Years Of Experience In Behavioral Health Case Management Or Related Field

  • Knowledge Of Mental Health Disorders And Treatment Options

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Clients, Healthcare Providers, And Other Stakeholders

  • Ability To Navigate And Utilize Electronic Health Record Systems To Document And Track Client Progress And Treatment Plans.
