- Coordinate and manage the care of oncology and transplant patients to ensure high-quality, patient-centered care.
- Utilize clinical expertise to assess patient needs and develop individualized care plans.
- Communicate effectively with patients, families, and healthcare team members to ensure continuity of care and patient satisfaction.
- Act as a patient advocate, addressing any concerns or issues that may arise during the course of treatment.
- Collaborate with physicians and other healthcare providers to ensure timely and appropriate care for patients.
- Monitor and evaluate patient progress and adjust care plans as needed.
- Educate patients and their families on treatment plans, medications, and self-care techniques.
- Maintain accurate and thorough documentation of patient care and progress.
- Stay current on advancements in oncology and transplant treatments and incorporate new knowledge into patient care plans.
- Ensure compliance with all regulatory and organizational standards.
- Participate in multidisciplinary team meetings and contribute to the development of patient care protocols.

Registered Nurse (Rn) License: A Candidate For This Position Must Possess A Valid Rn License In The State Where The Job Is Located. This License Should Be In Good Standing And Free Of Any Disciplinary Actions.
Oncology And Transplant Experience: The Ideal Candidate Should Have At Least 2-3 Years Of Experience Working In An Oncology And/Or Transplant Setting. This Will Ensure That They Have A Strong Understanding Of The Unique Needs And Challenges Of These Patient Populations.
Case Management Certification: Cvs Health Prefers Candidates Who Have Obtained Certification As A Case Manager (Ccm) Or In Oncology Nursing (Ocn). This Demonstrates A Commitment To Ongoing Education And Professional Development In The Field.
Knowledge Of Medicare And Medicaid Regulations: As A Case Manager Rn, The Candidate Will Be Responsible For Coordinating Care And Services For Patients With Complex Medical Needs. A Strong Understanding Of Medicare And Medicaid Regulations Is Essential To Ensure Compliance And Provide The Best Care For Patients.
