Case Manager RN

We are seeking an experienced and highly motivated Case Manager RN to join our CVS Health team. The successful candidate will be responsible for assessing, planning, coordinating, and monitoring an individual's care and services, to ensure the highest quality of care and cost-effectiveness. This is an excellent opportunity for a qualified registered nurse to use their knowledge and skills to provide support and care to patients in our facility.The ideal candidate will have a current Registered Nurse (RN) license from an accredited program; a minimum of two years of experience in case management or discharge planning; excellent communication and organizational skills; and the ability to work both independently and in a team setting.
