- Conduct thorough assessments of patients' medical needs and make appropriate referrals to specialists and other healthcare professionals.
- Utilize electronic medical records and other systems to document patient care and track progress.

- Collaborate with community resources and organizations to connect patients with additional support and services.
- Assist patients in navigating the healthcare system and understanding their treatment options.

- Maintain confidentiality and adhere to all HIPAA regulations in handling patient information.
- Stay updated on industry developments and best practices in case management.

Bachelor's Degree In Healthcare Administration, Social Work, Or A Related Field.
Minimum Of 3 Years Experience In Case Management Or Care Coordination.
Knowledge Of Healthcare Policies, Regulations, And Procedures.
Strong Communication And Interpersonal Skills, With The Ability To Build Relationships With Patients, Providers, And Other Stakeholders.
Proficiency In Electronic Medical Record Systems And Microsoft Office.
