- Act as a resource and guide for members in navigating the complexities of the healthcare system, including insurance coverage, medication management, and healthcare options.
- Conduct thorough assessments of members' needs and create personalized care plans to help them achieve their health goals.
- Monitor and document members' progress and make adjustments to care plans as needed.
- Identify and escalate any potential issues or concerns to the appropriate parties for resolution.
- Maintain confidentiality and adhere to privacy regulations when handling sensitive member information.
- Continuously seek opportunities to improve the member experience and streamline processes within the Engagement Hub.
Strong Communication And Interpersonal Skills
Bachelor's Degree In Social Work, Nursing, Or A Related Field
Minimum Of 2 Years Of Experience In Care Management Or Patient Engagement
Knowledge Of Healthcare Industry And Ability To Navigate Insurance And Benefit Plans
