Community Health Worker

Responsibilities:

1. Provide direct healthcare services to individuals and families in our community.
2. Utilize public health principles and practices in the implementation of healthcare services.
3. Develop and maintain relationships with community members and organizations.
4. Ensure compliance with all relevant laws, regulations, and policies.
5. Provide education and outreach to underserved populations in the community.
6. Develop and implement health-promotion and disease-prevention strategies.
7. Participate in health assessments and screenings.
8. Collaborate with healthcare professionals and other stakeholders on community health initiatives.
9. Monitor and evaluate the effectiveness of healthcare programs and services.
10. Assist with grant writing and fundraising initiatives.
