Patient Care Coordinator

Concord Hospital is seeking a highly organized, compassionate, and detail-oriented Patient Care Coordinator to join our team! In this role, you will be responsible for providing excellent customer service to patients and families by ensuring that their care and experience exceeds their expectations. You will need to demonstrate strong critical thinking skills, as well as excellent interpersonal and communication skills in order to collaborate with a wide range of healthcare professionals. The ideal candidate will have a minimum of five years of healthcare experience, as well as a Bachelor’s degree in a related field.
