Reg Dietitian Ambulatory

The Community Health Network is seeking an experienced Registered Dietitian Ambulatory to join our team. This role plays an essential part in providing high-quality nutrition care and education to our patients and their families. The successful candidate will be an energetic, self-motivated professional with a passion for helping others maintain a healthy lifestyle.The ideal candidate will have a Bachelor’s degree in Nutrition and Dietetics or equivalent, with at least five years of clinical experience in this field. They must be registered with the Academy of Nutrition and Dietetics and hold a valid state license. Additionally, the successful candidate must be passionate about working with diverse populations, have excellent communication and interpersonal skills, and be able to work independently with minimal supervision.

Responsibilities:

• Develop and maintain patient care plans to promote healthy nutrition and lifestyles
• Provide nutrition counseling and education to patients and their families
• Monitor patient progress and adjust plans as needed
• Collaborate with other healthcare professionals to coordinate care plans
• Research and stay up-to-date on nutrition and dietetics best practices
• Participate in community events to promote healthy nutrition habits
• Maintain accurate records and documentation of patient care
• Adhere to all applicable laws, regulations, and professional standards
