Patient Care Coordinator

At Community Health Network, we are looking for an experienced and compassionate Patient Care Coordinator to join our team. The successful candidate will provide support to patients and families in developing and implementing a plan of care and to coordinate the patient’s care and services. The Patient Care Coordinator will work with the healthcare team to ensure that the patient and family goals are met in a timely, efficient, and patient-centered manner.We are looking for someone who has excellent communication skills, strong organizational skills, and the ability to work well with a multi-disciplinary team. The ideal candidate should demonstrate a passion for providing quality patient care, have a strong desire to serve others, and possess the ability to work independently and with a team. To be successful in this role, the candidate must have a minimum of 2 years' experience in a hospital or health organization setting and a Bachelor's degree in a related field.

Responsibilities:
