Responsibilities:

• Document patient medication and reconcile discrepancies between medication lists
• Ensure all medication data is accurately recorded, updated, and stored in patient records
• Maintain patient medication profiles and compliance with medication orders
• Assist with data entry of patient medication records
• Participate in medication reconciliation processes and procedures
• Monitor and report medication errors
• Resolve discrepancies between medication lists and reports
• Assist with the implementation of medication safety initiatives
• Collaborate with healthcare teams to ensure proper medication use
• Communicate with patients and their families regarding medication-related topics
• Participate in continuing education and professional development activities as needed
