
LPN - Behavioral Health
We are looking for a compassionate Licensed Practical Nurse (LPN) to join our Behavioral Health team at Community Health Network. The ideal candidate will have a strong commitment to providing quality patient care and uphold our organizational values of integrity, respect, trust, excellence, and innovation.This position requires a valid LPN license and at least 2 years of relevant experience. The successful candidate will possess excellent communication and organizational skills, have a keen eye for detail, and be able to work independently and as part of a team. The LPN - Behavioral Health will be responsible for providing direct patient care, managing patient records, and coordinating care with other healthcare team members. Responsibilities:
- Provide direct patient care according to established standards and practices.
- Manage patient records and documentation accurately and in a timely manner.
- Coordinate patient care with other healthcare team members including physicians, nurses, therapists, and other specialists.
- Monitor the condition of patients and report any changes to the supervising physician.
- Administer medications and treatments as prescribed.
- Educate patients and families on health care needs.
- Provide emotional support and guidance to patients and families.
- Participate in health promotion activities.
- Respect patient confidentiality.
- Follow all safety and infection control policies.
Cpr Certification
Current Unencumbered License To Practice As A Licensed Practical Nurse In The State Of Indiana
Completed An Accredited Practical Nursing Program
