LPN - Behavioral Health

We are looking for a compassionate Licensed Practical Nurse (LPN) to join our Behavioral Health team at Community Health Network. The successful candidate will possess excellent communication and organizational skills, have a keen eye for detail, and be able to work independently and as part of a team. The LPN - Behavioral Health will be responsible for providing direct patient care, managing patient records, and coordinating care with other healthcare team members. Responsibilities:

1. Provide direct patient care according to established standards and practices.
2. Manage patient records and documentation accurately and in a timely manner.
3. Coordinate patient care with other healthcare team members including physicians, nurses, therapists, and other specialists.
4. Monitor the condition of patients and report any changes to the supervising physician.
5. Administer medications and treatments as prescribed.
6. Educate patients and families on health care needs.
7. Provide emotional support and guidance to patients and families.
8. Participate in health promotion activities.
9. Respect patient confidentiality.
10. Follow all safety and infection control policies.
