Job Description

We are looking for an experienced and motivated Population Health Product Program Manager to join CommonSpirit Health! The successful candidate will have the opportunity to develop and implement innovative population health products in order to improve patient outcomes and drive system-wide performance.The ideal candidate will have a passion for population health and an entrepreneurial spirit. They will have a proven track record of leading projects, be highly organized, and possess excellent communication and problem-solving skills.The Population Health Product Program Manager will be responsible for developing strategy and leading projects to support population health initiatives. They will have the opportunity to work with cross-functional teams to create and launch products, provide operational support, and collaborate on best practices.To be successful in this role, the ideal candidate must have a Bachelor's degree in a related field and at least 3 years of experience in population health and/or program management. Additionally, they will have a strong understanding of healthcare operations and experience leading projects from inception to completion. Responsibilities:

  1. Develop and implement innovative population health products to improve patient outcomes and drive system-wide performance.
  2. Work with cross-functional teams to create and launch products, provide operational support, and collaborate on best practices.
  3. Develop strategy and lead projects to support population health initiatives.
  4. Lead projects from inception to completion.
  5. Monitor and analyze population health trends to identify areas for improvement.
  6. Collaborate with internal and external stakeholders to ensure successful product launches.
  7. Provide technical support and guidance to team members.
  8. Develop and implement strategies to ensure regulatory compliance.
  9. Manage budget and resources to ensure efficient and effective product development and implementation.
  10. Prepare and present project reports to stakeholders.
