  1. Provide psychosocial support to patients undergoing transplant procedures and their families.
  2. Build strong relationships with patients in order to understand and address their emotional and social needs.
  3. Help patients navigate the challenges of the transplant process and provide resources and support.
  4. Assess and address any potential barriers to successful transplant outcomes, such as financial, emotional, or social factors.
  5. Collaborate with the multidisciplinary transplant team to develop comprehensive care plans for patients.
  6. Educate patients and their families about the emotional and social impact of organ transplantation.
  7. Advocate for patients to ensure their needs are met throughout the transplant process.
  8. Conduct psychosocial evaluations and assessments to identify any potential mental health concerns and provide appropriate interventions.
  9. Document all patient interactions and progress in accordance with hospital policies and procedures.
  10. Stay up-to-date on current transplant care practices and participate in ongoing education and training opportunities.
  11. Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
  12. Serve as a supportive resource for other members of the healthcare team, such as physicians, nurses, and case managers.
  13. Provide crisis intervention and support for patients and their families during difficult times.
  14. Collaborate with community resources and organizations to provide additional support for patients and their families.
  15. Adhere to all state and federal regulations and guidelines related to social work practice.

Job Qualifications
  • Master's Degree In Social Work From An Accredited University: Candidates For A Transplant Social Worker Position At Cleveland Clinic Should Have A Master's Degree In Social Work From A Reputable And Accredited University.
