- Provide compassionate and specialized care to patients in need in a hospice setting.
- Deliver exceptional patient care by assessing and addressing the physical, emotional, and spiritual needs of patients.
- Utilize previous experience in hospice care to provide high-quality and knowledgeable care to patients.
- Demonstrate empathy and understanding towards patients and their families during difficult times.
- Collaborate with a multidisciplinary team to develop and implement individualized care plans for patients.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' conditions and report any changes to the appropriate healthcare providers.
- Educate patients and their families on the hospice care process and provide emotional support.
- Ensure accurate and timely documentation of patient care and progress.
- Maintain a safe and comfortable environment for patients and their families.
- Adhere to all hospice care regulations and guidelines.
- Continuously strive to improve patient outcomes and satisfaction.
- Attend and participate in meetings, trainings, and educational opportunities to enhance skills and knowledge.
- Serve as a professional and ethical representative of Cleveland Clinic at all times.
- Collaborate with community resources to ensure continuity of care for patients.
- Maintain confidentiality of patient information and adhere to HIPAA regulations.
- Perform other duties as assigned by the hospice care team or supervisor.
Valid Rn License In The State Of Ohio
Bachelor's Degree In Nursing (Bsn) From An Accredited Institution
Minimum Of 2 Years Of Nursing Experience, Preferably In Hospice Or Palliative Care
Certification In Hospice And Palliative Care (Hpcc) Strongly Preferred
Strong Communication And Interpersonal Skills, With The Ability To Provide Emotional Support To Patients And Their Families During End-Of-Life Care.
