  1. Provide pharmaceutical care to patients on a per diem basis, ensuring safe and effective use of medications.
  2. Collaborate with healthcare professionals to develop and implement individualized treatment plans for patients.
  3. Review and verify medication orders for accuracy and appropriateness.
  4. Monitor patient response to medication therapy and make necessary adjustments.
  5. Maintain accurate and complete patient records, including medication profiles and drug interactions.
  6. Educate patients and caregivers on proper medication use, potential side effects, and drug interactions.
  7. Participate in quality assurance and improvement initiatives to ensure high-quality patient care.
  8. Stay up-to-date on new medications, therapies, and treatment guidelines.
  9. Adhere to all state and federal regulations related to pharmacy practice.
  10. Demonstrate professionalism and excellent customer service when interacting with patients, colleagues, and other healthcare professionals.
  11. Collaborate with other healthcare professionals to develop and implement medication safety protocols.
  12. Provide training and guidance to pharmacy technicians and other staff members.
  13. Serve as a resource for drug information and participate in medication therapy management programs.
  14. Maintain a clean and organized work environment in accordance with departmental standards.
  15. Attend departmental meetings and participate in professional development opportunities to enhance skills and knowledge.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School

  • Active Pharmacist License In The State Of Ohio

  • Minimum Of 2 Years Experience In A Clinical Pharmacy Setting

  • Knowledge Of Current Pharmacy Practices, Laws, And Regulations

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Healthcare Team And Patients
