  1. Coordinate and facilitate the care of oncology patients throughout their treatment journey, including scheduling appointments, arranging consultations, and coordinating with other healthcare providers.
  2. Serve as a liaison between patients, their families, and the healthcare team, ensuring effective communication and collaboration.
  3. Assess patient needs and develop personalized care plans in collaboration with the healthcare team and patients' families.
  4. Educate patients and their families about their diagnosis, treatment options, and potential side effects, and provide them with the necessary resources and support.
  5. Monitor and track patients' progress and assist in managing any ongoing symptoms or complications.
  6. Collaborate with insurance companies and other organizations to ensure patients have access to necessary treatments and resources.
  7. Maintain accurate and up-to-date patient records and documentation.
  8. Stay current with advancements in oncology treatments and participate in ongoing education and training.
  9. Provide emotional support and counseling to patients and their families during their cancer journey.
  10. Participate in multidisciplinary team meetings to discuss patient cases and develop comprehensive treatment plans.
  11. Advocate for patients' needs and rights, ensuring they receive the highest quality of care.
  12. Adhere to all ethical, legal, and regulatory standards and maintain patient confidentiality at all times.
  13. Contribute to the development and implementation of quality improvement initiatives to enhance the delivery of oncology care.
  14. Uphold the values and mission of Cleveland Clinic and promote a positive and welcoming environment for patients, families, and colleagues.
  15. Continuously evaluate and improve processes to enhance the overall patient experience and outcomes.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Field, With At Least 2 Years Of Experience In Oncology Care Coordination.

  • Knowledge Of Cancer Treatment And Management Protocols, Including Chemotherapy, Radiation Therapy, And Immunotherapy.

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Multidisciplinary Teams And Communicate With Patients And Families.

  • Experience With Electronic Medical Records And Care Coordination Software.

  • Current Certification In Oncology Nursing (Ocn) Preferred.
