
Community Health Worker

- Develop and maintain strong relationships with community members, leaders, and organizations to effectively promote and deliver community health programs and services.
- Conduct outreach and engagement activities to identify and connect with individuals and families in need of support and resources.
- Conduct comprehensive assessments to identify the health needs and goals of individuals and families, and develop personalized care plans to address these needs.
- Provide education and support to individuals and families on health promotion, disease prevention, and management of chronic conditions.
- Connect individuals and families with community resources, such as healthcare providers, social services, and support groups, to help them achieve their health goals.
- Collaborate with healthcare providers and other professionals to ensure coordinated and comprehensive care for individuals and families.
- Assist in the planning, implementation, and evaluation of community health programs and initiatives.
- Maintain accurate and up-to-date records of all interactions and services provided to individuals and families.
- Serve as a liaison between the community and Cleveland Clinic, advocating for the needs and concerns of community members.
- Continuously monitor and evaluate the effectiveness of community health programs and services, and make recommendations for improvement.
- Stay informed about current community health issues and trends, and use this knowledge to inform program development and service delivery.
- Adhere to all ethical and professional standards in the delivery of services and interactions with community members.
- Participate in training and professional development opportunities to enhance knowledge and skills in community health and related areas.
- Represent Cleveland Clinic in a positive and professional manner at all times, promoting its values and mission to improve the health and well-being of the community.
Bachelor's Degree In Public Health, Community Health, Or Related Field.
Certification As A Community Health Worker Or Equivalent Experience In A Healthcare Setting.
Knowledge Of Community Resources, Social Services, And Healthcare Systems.
Excellent Communication And Interpersonal Skills, With The Ability To Build Rapport With Diverse Populations.
Experience In Conducting Health Education And Outreach Programs, And Ability To Organize And Facilitate Community Events.
