
Clinical Pharmacist

- Review and assess patient medication regimens to ensure safety, appropriateness, and effectiveness.
- Collaborate with medical staff to develop treatment plans that include medication therapy.
- Monitor patients for adverse drug reactions and make appropriate recommendations for therapy adjustments.
- Educate patients and healthcare professionals on medication usage, side effects, and potential interactions.
- Participate in multidisciplinary rounds and provide medication-related recommendations to healthcare team.
- Maintain accurate and detailed patient medication records.
- Stay updated on current pharmaceutical research, drug therapies, and treatment guidelines.
- Ensure compliance with all federal, state, and institutional regulations and policies.
- Participate in quality improvement initiatives and assist with medication-related audits.
- Serve as a resource for drug information questions and provide guidance on medication management.
- Collaborate with pharmacy team to ensure proper storage, handling, and dispensing of medications.
- Contribute to medication safety initiatives and participate in adverse drug event reporting.
- Communicate effectively with patients, families, and healthcare team members.
- Participate in the training and mentoring of pharmacy students and residents.
- Adhere to ethical and professional standards in all aspects of job performance.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School.
Active Pharmacist License In The State Of Ohio.
Completion Of A Post-Graduate Residency Program In Clinical Pharmacy.
Board Certification In A Relevant Area Of Practice, Such As Ambulatory Care Or Critical Care.
At Least 2-3 Years Of Experience Working As A Clinical Pharmacist In A Hospital Or Healthcare Setting.
