1. Review and assess patient medication regimens to ensure safety, appropriateness, and effectiveness.
2. Collaborate with medical staff to develop treatment plans that include medication therapy.
3. Monitor patients for adverse drug reactions and make appropriate recommendations for therapy adjustments.
4. Educate patients and healthcare professionals on medication usage, side effects, and potential interactions.
5. Participate in multidisciplinary rounds and provide medication-related recommendations to healthcare team.
6. Maintain accurate and detailed patient medication records.

9. Participate in quality improvement initiatives and assist with medication-related audits.
10. Serve as a resource for drug information questions and provide guidance on medication management.
11. Collaborate with pharmacy team to ensure proper storage, handling, and dispensing of medications.
12. Contribute to medication safety initiatives and participate in adverse drug event reporting.
13. Communicate effectively with patients, families, and healthcare team members.
14. Participate in the training and mentoring of pharmacy students and residents.
15. Adhere to ethical and professional standards in all aspects of job performance.
