- Dispense medication and provide medication therapy management to patients according to established protocols and procedures.
- Collaborate with healthcare providers to develop and implement treatment plans that optimize patient outcomes.
- Ensure accurate and timely delivery of medications to patients and provide counseling on proper medication use.
- Educate patients on potential side effects and drug interactions, as well as ways to manage their medication regimen.
- Monitor patient response to medication therapy and make appropriate adjustments as needed.
- Maintain accurate and up-to-date patient records and documentation.
- Adhere to all state and federal laws and regulations related to pharmacy practice.
- Keep current with new medications and treatment protocols through continuous education and training.
- Work closely with other healthcare professionals to develop and implement medication safety protocols.
- Assist with inventory management, medication ordering, and budgeting to ensure efficient and cost-effective pharmaceutical care.
- Provide guidance and support to pharmacy technicians and other staff members.
- Maintain a high level of professionalism and customer service in all interactions with patients and healthcare providers.
- Participate in quality improvement initiatives and contribute to the development of evidence-based pharmacy practices.

Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School
Current Pharmacist Licensure In The State Of Ohio
Completion Of A Post-Graduate Residency Program In Clinical Pharmacy
Board Certification In A Relevant Specialty (E.g. Ambulatory Care, Critical Care)
Minimum Of 3 Years Of Experience In A Clinical Pharmacy Role In A Hospital Or Healthcare Setting
