  1. Provide safe and effective medication therapy management for patients.
  2. Collaborate with physicians, nurses, and other healthcare professionals to develop and implement patient care plans.
  3. Ensure compliance with all legal and regulatory requirements pertaining to pharmacy practice.
  4. Conduct medication reconciliation and review for accuracy and appropriateness.
  5. Monitor patients for adverse drug reactions and provide appropriate interventions.
  6. Educate patients and their families on proper medication use and potential side effects.
  7. Participate in the development and implementation of pharmacy policies and procedures.
  8. Maintain accurate and detailed patient medication records.
  9. Participate in multidisciplinary rounds and provide medication recommendations.
  10. Stay up-to-date on new medications, drug interactions, and other relevant pharmacy information.
  11. Provide drug information and consultation to healthcare professionals.
  12. Communicate effectively with patients, healthcare professionals, and other team members.
  13. Utilize critical thinking skills to identify and resolve medication-related issues.
  14. Work efficiently and effectively in a fast-paced healthcare environment.
  15. Continuously assess and improve medication therapy management processes.
  16. Participate in quality improvement initiatives to enhance patient outcomes.
  17. Maintain a high level of professionalism and ethical standards.
  18. Attend and participate in continuing education programs to maintain licensure and stay current on best practices in pharmacy.
  19. Support and promote the mission, vision, and values of Cleveland Clinic.
  20. Provide exceptional customer service to patients and their families.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree: A Clinical Pharmacist At Cleveland Clinic Must Possess A Doctor Of Pharmacy (Pharmd) Degree From An Accredited School Of Pharmacy.

  • State Licensure: The Candidate Must Be Licensed To Practice Pharmacy In The State Of Ohio And Maintain An Active License Throughout Their Employment.

  • Board Certification: Cleveland Clinic Prefers Clinical Pharmacists Who Are Board-Certified In A Specialty Area Such As Pharmacotherapy Or Ambulatory Care.

  • Residency Training: A Completed Pharmacy Practice Residency Or Equivalent Experience Is Desired To Demonstrate Advanced Clinical Knowledge And Skills.

  • Strong Communication And Teamwork Skills: The Clinical Pharmacist Must Possess Excellent Communication Skills To Collaborate With Other Healthcare Professionals And Effectively Communicate With Patients. They Should Also Be Able To Work Well In A Multidisciplinary Team Setting.
