  1. Conduct initial assessments of patients to determine their social, emotional, and psychological needs.
  2. Develop and implement individualized care plans for patients in collaboration with interdisciplinary teams.
  3. Provide counseling and support to patients and their families to address social, emotional, and psychological challenges.
  4. Coordinate and facilitate communication between patients, families, healthcare providers, and community resources.
  5. Conduct regular follow-up assessments to evaluate the effectiveness of the care plan and make necessary adjustments.
  6. Advocate for patients' rights and ensure they have access to necessary healthcare services and resources.
  7. Collaborate with insurance companies and other third-party payers to ensure coverage for necessary services.
  8. Maintain accurate and up-to-date documentation of patient interactions, care plans, and progress.
  9. Provide education and resources to patients and their families about available community resources and support services.
  10. Participate in interdisciplinary team meetings and provide input on patients' progress and needs.
  11. Stay informed about changes and updates in policies, procedures, and regulations related to case management and social work.
  12. Maintain a high level of professionalism and ethical standards in all interactions with patients, families, and colleagues.
  13. Collaborate with other healthcare professionals to develop and implement policies and programs that promote the well-being of patients.
  14. Participate in ongoing training and professional development opportunities to enhance knowledge and skills in social work and case management.
  15. Uphold the mission, vision, and values of Cleveland Clinic in all aspects of the job.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited Institution.

  • Minimum Of 2 Years Of Experience In A Social Work Or Case Management Role, Preferably In A Healthcare Setting.

  • Knowledge Of Local Resources And Community Services, As Well As Experience In Coordinating Care And Making Referrals.

  • Excellent Communication And Interpersonal Skills To Effectively Work With Patients, Families, And Healthcare Team Members.

  • Ability To Multitask And Manage A Diverse Caseload, While Maintaining Accurate And Timely Documentation Of Patient Interactions And Progress.
