  1. Coordinate and manage care for cardiac patients, ensuring they receive the best possible treatment and support throughout their journey.

  3. Conduct assessments to identify the specific needs and goals of each patient and make appropriate referrals to specialists or other healthcare providers as needed.
  4. Collaborate with the Cardiology team to ensure seamless and efficient delivery of care.
  5. Monitor and track patient progress, follow up on treatment plans, and make necessary adjustments as needed.
  6. Educate patients on their condition, treatment plan, and self-management strategies.
  7. Advocate for patients and address any concerns or questions they may have.
  8. Maintain accurate and organized patient records, including documentation of all care coordination activities.
  9. Stay up-to-date on the latest developments and advancements in cardiology to provide the best possible care to patients.
  10. Participate in quality improvement initiatives and contribute to the development of care coordination protocols and policies.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Field

  • Minimum Of 3 Years Experience In A Clinical Setting, Preferably In Cardiology

  • Strong Knowledge Of Cardiology Treatments, Procedures, And Medications

  • Experience In Care Coordination, Case Management, Or Patient Navigation

  • Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With A Multidisciplinary Team And Effectively Educate Patients And Their Families About Their Care Plan.
