Community Services Representative

The City of Pasadena Public Health Department is currently seeking an enthusiastic and skilled Community Services Representative I to support HIV program services. This position works with clients living with HIV in a variety of settings, including in the field, and is primarily responsible for assisting clients navigate through a variety of systems with the support of a team of case managers and/or social workers. Provides linkage and partner services to newly diagnosed people living with HIV, including warm handoff to medical services, partner testing, and other supportive services. Assists in coordinating and participates in targeted outreach activities, such as wellness fairs, street outreach, and other special events. Participates in peer support, HIV education, and risk reduction counseling; acts as client advocate as needed. Assists in navigation through HIV care continuum and conducts efforts to retain or re-engage people in care.
