Execute scope of program and association of projects in collaboration leadership and stakeholders; design work plans and establishes deliverables that support organizational goals for practice transformation. Design, develop, evaluate and support the implementation of team-driven performance improvement initiatives. Job Qualifications

  • 5 years Experience in large matrixed health care organizations

  • Knowledge of health care information system interfaces and electronic technology

  • NCQA PCMH Content Expert Certification preferred

  • Demonstrated ability to develop project task lists and execute to organizational timelines
