- Provide exceptional pharmaceutical care to pediatric patients by accurately dispensing and monitoring medications according to established protocols and guidelines.
- Collaborate with the healthcare team to develop and implement individualized treatment plans for pediatric patients.
- Utilize strong clinical knowledge and expertise to ensure safe and effective medication use for pediatric patients.
- Stay up-to-date with current developments and advancements in pediatric pharmacy practice to provide the best possible care.
- Conduct medication reconciliation and review to identify and prevent potential drug interactions or adverse reactions in pediatric patients.
- Communicate with patients and their families to educate them about their medications and address any concerns or questions.
- Serve as a resource for healthcare professionals, providing drug information and recommendations for pediatric patients.
- Participate in multidisciplinary rounds and meetings to discuss patient progress and medication-related issues.
- Assist in the development and implementation of policies and protocols related to pediatric pharmacy services.
- Maintain accurate and complete patient records and documentation in compliance with regulatory standards.
- Contribute to quality improvement initiatives and research projects to improve patient outcomes.
- Foster a culture of teamwork and collaboration among pharmacy staff and other healthcare professionals.
- Adhere to all safety and infection control policies and procedures.
- Participate in ongoing education and training to maintain licensure and keep current with best practices in pediatric pharmacy.
- Serve as a role model and mentor to other pharmacy staff members, promoting a culture of continuous learning and professional growth.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution.
Completion Of A Post-Graduate Pharmacy Residency Program With A Focus On Clinical Pharmacy Practice.
Active Pharmacist License In The State Of Ohio.
Board Certification In A Relevant Area Of Practice Such As Pharmacotherapy, Pediatrics, Or Critical Care.
At Least 3-5 Years Of Experience In A Clinical Pharmacy Role, Preferably In A Pediatric Setting.
