
Community Health Worker
At Cincinnati Children's, we are looking for a vibrant, passionate, and dedicated Community Health Worker to join our team. The Community Health Worker will play an integral role in providing quality healthcare with an emphasis on promoting health education and preventive care to the underserved population in our communities. We are searching for a highly motivated individual who is passionate about making a positive impact on the health of our community. The ideal candidate should have a Bachelor's Degree in Health Science or a related field, as well as at least two years of experience in community health in the Cincinnati area. In addition, the successful candidate will possess strong communication and interpersonal skills, as well as the ability to work independently and as part of a team. The ideal candidate should also demonstrate a commitment to social justice and equity, as well as experience working with diverse populations.
Responsibilities:
- Provide quality healthcare with an emphasis on promoting health education and preventive care to underserved populations.
- Develop and implement health education programs for the community.
- Collaborate with other health professionals to ensure quality healthcare.
- Liaise with community organizations, agencies, and health departments.
- Participate in outreach activities and health fairs to promote community health.
- Identify and connect individuals with community resources and services.
- Monitor and evaluate program performance and outcomes.
- Maintain accurate and up-to-date records of program activities.
- Develop and maintain positive relationships with community members.
- Advocate for social justice and equity within the community.
