- Provide case management services to clients by assessing their needs and developing individualized care plans.

Master's Degree In Social Work (Msw) From An Accredited Program
Current And Valid Licensed Social Worker (Lsw) Or Licensed Clinical Social Worker (Lcsw) Certification
Minimum Of 2 Years Of Experience In Social Work Or Case Management, Preferably In A Healthcare Setting
Knowledge Of And Experience With Case Management Principles And Practices, Including Assessment, Care Planning, And Coordination Of Services
Excellent Communication Skills, Both Verbal And Written, And The Ability To Work Effectively With Diverse Populations And Interdisciplinary Teams.
