  1. Provide case management services to clients by assessing their needs and developing individualized care plans.
  2. Conduct thorough evaluations and assessments to identify client's strengths, needs, and resources.
  3. Collaborate with medical professionals, community resources, and other team members to ensure comprehensive and coordinated care.
  4. Monitor client progress and make necessary adjustments to care plans as needed.
  5. Advocate for clients and connect them with appropriate resources and services.
  6. Educate clients and their families on available healthcare options and resources.
  7. Maintain accurate and up-to-date records of client interactions and progress.
  8. Adhere to all ethical and legal standards in the provision of services.
  9. Participate in team meetings and case conferences to discuss client progress and collaborate on treatment plans.
  10. Stay current on industry trends and best practices in case management and social work.
  11. Provide crisis intervention and support as needed.
  12. Maintain confidentiality of all client information and adhere to HIPAA guidelines.
  13. Uphold the values and mission of Cigna in all interactions with clients and colleagues.
  14. Communicate effectively and professionally with clients, colleagues, and other healthcare professionals.
  15. Continuously strive to improve the quality of care and services provided to clients.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited Program

  • Current And Valid Licensed Social Worker (Lsw) Or Licensed Clinical Social Worker (Lcsw) Certification

  • Minimum Of 2 Years Of Experience In Social Work Or Case Management, Preferably In A Healthcare Setting

  • Knowledge Of And Experience With Case Management Principles And Practices, Including Assessment, Care Planning, And Coordination Of Services

  • Excellent Communication Skills, Both Verbal And Written, And The Ability To Work Effectively With Diverse Populations And Interdisciplinary Teams.
