- Provide exceptional care and support to Medicare members who are in need of transplant care management.
- Utilize expertise in transplant care management to develop and implement individualized care plans for Medicare members.
- Monitor and assess the health and well-being of Medicare members, and make necessary adjustments to care plans as needed.
- Maintain a deep understanding of Medicare policies and regulations to ensure compliance and provide accurate information to Medicare members.
- Collaborate with healthcare providers and other members of the healthcare team to coordinate and optimize care for Medicare members.
- Serve as a resource for Medicare members and their families, providing education and guidance on transplant care management.
- Identify and address any barriers or challenges that may impact the health and well-being of Medicare members, and work to find solutions.
- Keep detailed and accurate records of all interactions and care provided to Medicare members.
- Continuously strive to improve the quality of care provided to Medicare members, staying up-to-date on industry developments and best practices.

Bachelor's Degree In Nursing, Healthcare Administration, Or Related Field
Minimum Of 3 Years Of Experience In Transplant Care Management Or Related Field
Knowledge Of Medicare Regulations And Guidelines Related To Transplant Care
Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Teams
Current Licensure Or Certification As A Registered Nurse (Rn) Or Other Healthcare Professional (E.g. Social Worker, Case Manager)
