  1. Analyze Medicare member data to identify trends, gaps in care, and opportunities for improvement.
  2. Develop and implement strategic plans and interventions to improve healthcare outcomes for Medicare population.
  3. Utilize clinical expertise to provide guidance and support to case management team.
  4. Collaborate with cross-functional teams to develop and implement strategies for improving Medicare member health.
  5. Communicate with Medicare members and their families to assess needs and develop personalized care plans.
  6. Monitor and evaluate Medicare member progress and make adjustments to care plans as needed.
  7. Stay up-to-date on Medicare regulations and policies to ensure compliance and adherence to standards of care.
  8. Educate and train other team members on Medicare population health best practices and strategies.
  9. Utilize data analysis and reporting to track and measure the success of population health initiatives.
  10. Serve as a liaison between Cigna and external partners, such as healthcare providers and community organizations, to promote collaboration and coordination of care for Medicare members.
  11. Continuously seek opportunities for process improvement and efficiency within the Medicare population health program.
  12. Provide support and guidance to team members on complex Medicare cases.
  13. Maintain accurate and detailed documentation of all interactions and interventions with Medicare members.
  14. Represent Cigna in a professional manner and uphold the company's values and mission to improve the health and well-being of our Medicare members.

Job Qualifications
  • Bachelor's Or Master's Degree In Nursing Or A Related Field

  • Active Rn License In The State Where The Position Is Located

  • Minimum Of 3-5 Years Of Clinical Experience In A Healthcare Setting, Preferably With Medicare Patients

  • Experience In Case Management, Care Coordination, Or Population Health Management

  • Strong Analytical And Problem-Solving Skills, With The Ability To Use Data To Inform Decision-Making And Drive Results.
