  1. Provide compassionate and exceptional care to pediatric patients and their families.
  2. Collaborate with a multidisciplinary team to ensure successful transitions from inpatient to outpatient care.
  3. Utilize strong critical thinking skills to assess and address patient needs during the transition process.
  4. Communicate effectively with patients, families, and healthcare professionals to ensure proper care coordination.
  5. Maintain a current California RN license and adhere to all state and organizational regulations and policies.
  6. Utilize experience and knowledge of acute care to provide high-quality care to patients.
  7. Develop and implement care plans to meet the individual needs of each patient.
  8. Provide education and support to patients and their families during the transition process.
  9. Document patient care and progress accurately and in a timely manner.
  10. Participate in continuous education and training to stay up-to-date with best practices in pediatric care.
  11. Adhere to CHOC Children's Hospital's mission and values of providing the highest level of pediatric care.
  12. Maintain a positive and professional demeanor while representing CHOC Children's Hospital.
  13. Act as an advocate for patients and their families to ensure their needs are met.
  14. Uphold patient confidentiality and privacy at all times.

Job Qualifications
  • Bachelor's Degree In Nursing: A Transition Rn Care Manager - Outpatient At Choc Children's Hospital Must Have A Bachelor's Degree In Nursing From An Accredited Institution. This Educational Background Provides The Necessary Foundation In Nursing Theory, Patient Care, And Clinical Practice.

  • Active Rn License In The State Of California: The Candidate Must Possess A Current And Active Registered Nurse (Rn) License In The State Of California. This License Ensures That The Individual Is Qualified And Authorized To Practice Nursing Within The State.

  • Minimum Of 3 Years Of Clinical Nursing Experience: Choc Children's Hospital Requires A Minimum Of 3 Years Of Clinical Nursing Experience For A Transition Rn Care Manager - Outpatient Position. This Ensures That The Candidate Has The Necessary Knowledge And Skills To Provide High-Quality Patient Care.

  • Experience In Pediatric Or Outpatient Setting: Experience In A Pediatric Or Outpatient Setting Is Highly Preferred For This Role. This Experience Demonstrates The Candidate's Understanding Of The Unique Needs And Challenges Of Caring For Children In An Outpatient Setting.

  • Case Management Certification: A Certification In Case Management, Such As The Commission For Case Manager Certification (Ccmc), Is Preferred For This Role. This Certification Demonstrates The Candidate's Expertise In Coordinating And Managing Patient Care, Which Is A Crucial Aspect Of The Transition Rn Care Manager - Outpatient Position At Choc Children's Hospital.
