Community Health Navigator

Children's Hospital of Wisconsin is looking for an enthusiastic, passionate, and experienced Community Health Navigator to join our team. This is an exciting opportunity to help create healthier communities and make a direct impact on the lives of our patients and their families. The Community Health Navigator will work collaboratively with a multidisciplinary team to provide health navigation services, build relationships with community partners, and participate in developing and implementing strategies to improve health outcomes. We are looking for an individual with a strong commitment to health equity and a passion for working with diverse populations.Required qualifications include: • Bachelor’s Degree in Public Health, Health Education, Community Health, or a related field • At least two years of experience in health education, community health, social work, or a related field • Knowledge of community resources, public health, and health disparities • Excellent communication skills • Demonstrated ability to work both independently and as part of a team • Valid driver’s license and ability to travel as needed • Ability to work in a fast-paced environment with frequent interruptions If you are interested in joining us in making a difference in the health of Wisconsin children, we would love to hear from you.
