
Social Worker - Special Immunology

If so, Children's Hospital of Philadelphia (CHOP) is looking for a highly motivated, compassionate Social Worker to join our Special Immunology team.This is an exciting opportunity to work with a dedicated team of professionals, providing families with the specialized care and support they need. In this role, you will use your experience and skills to provide social work services to patients and families, as well as develop and implement treatment plans, and coordinate with other disciplines to ensure the best possible outcomes.The ideal candidate for this position will have a minimum of two years’ experience in pediatric social work, as well as a Master’s degree in Social Work from an accredited program. Responsibilities:
- Provide social work services to patients and families, including developing and implementing treatment plans.
- Coordinate with other disciplines to ensure the best possible outcomes.
- Facilitate education and support for families of children with special immunology needs.
- Actively participate in interdisciplinary team meetings and provide updates on patient progress.
- Assist in developing and maintaining community resources and referrals for families.
- Ensure compliance with all applicable laws, regulations, and standards of practice.
- Participate in professional development and continuing education activities.
- Maintain accurate and up-to-date records of patient assessments, treatment plans, and progress notes.
- Provide crisis intervention and supportive counseling to patients and families.
