
Social Worker - Inpatient Rehabilitation

We are currently seeking a compassionate and dedicated Social Worker to join our Inpatient Rehabilitation team. - Provide emotional support and counseling to pediatric patients and their families during their hospital stay.
- Collaborate with healthcare team members to assess the social and emotional needs of patients and develop individualized treatment plans.
- Conduct psychosocial assessments and develop interventions to address any barriers to the patient's recovery.
- Advocate for patients and families to ensure they receive the necessary resources and support for their physical, emotional, and social well-being.
- Coordinate and facilitate support groups and educational programs for patients and families.
- Collaborate with community agencies and resources to ensure a smooth transition for patients and families after discharge.
- Maintain accurate and thorough documentation of patient assessments, treatment plans, and progress notes.
- Provide crisis intervention and support to patients and families in difficult situations.
- Participate in interdisciplinary team meetings and provide input on patient care planning.
- Stay up-to-date on current social work practices and interventions to ensure the best possible care for patients and families.
- Act as a liaison between patients, families, and healthcare providers to promote effective communication and collaboration.
- Comply with all hospital policies and procedures, as well as ethical and professional standards for social work practice.
- Maintain confidentiality and adhere to HIPAA regulations when handling patient information.
- Uphold the mission and values of Children's Hospital of Philadelphia in all interactions with patients, families, and colleagues.
Bachelor's Or Master's Degree In Social Work From An Accredited University.
At Least 2 Years Of Experience In A Hospital Or Rehabilitation Setting, Preferably With Pediatric Patients.
Knowledge Of Child Development, Family Dynamics, And The Impact Of Illness Or Injury On A Child's Mental Health.
Strong Communication, Interpersonal, And Counseling Skills.
Current Licensure As A Social Worker In The State Of Pennsylvania (Or Eligibility To Obtain Licensure Within A Specified Time Frame).
