Job Description

We are currently seeking a compassionate and dedicated Social Worker to join our Inpatient Rehabilitation team. In this role, you will have the opportunity to work with children and their families to provide emotional support, advocacy, and resources during their rehabilitation journey.   1. Provide emotional support to children and their families during their rehabilitation journey.
  2. Advocate for the needs and rights of patients and their families.
  3. Assess the psychosocial needs of patients and develop individualized treatment plans.
  4. Collaborate with interdisciplinary team members to ensure comprehensive care for patients.
  5. Provide resources and referrals to patients and their families for ongoing support and services.
  6. Educate patients and families on coping strategies and community resources.
  7. Conduct assessments and screenings for child abuse and neglect.
  8. Maintain accurate and timely documentation of patient interactions and progress.
  9. Facilitate support groups and therapeutic activities for patients and families.
  10. Participate in discharge planning and coordination with community resources.
  11. Maintain confidentiality and adhere to ethical standards in all interactions.
  12. Stay updated on current social work practices and regulations.
  13. Develop and maintain relationships with community organizations and agencies.
  14. Collaborate with insurance companies and other third-party payers to ensure coverage for necessary services.
  15. Participate in team meetings and case conferences to discuss patient care and treatment plans.
  16. Communicate effectively with patients and families from diverse backgrounds.
  17. Demonstrate empathy, compassion, and patience in all interactions.
  18. Uphold the mission and values of Children's Hospital of Philadelphia.
  19. Adhere to all hospital policies and procedures.
  20. Continuously assess and evaluate the effectiveness of interventions and modify treatment plans as needed.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited Institution.

  • Minimum Of 2 Years Experience In A Healthcare Setting, Preferably In Inpatient Rehabilitation.

  • Knowledge Of Child Development, Family Systems, And Psychosocial Issues Related To Chronic Illness And Disability.

  • Strong Assessment And Counseling Skills, With The Ability To Develop And Implement Treatment Plans For Patients And Families.

  • Current State Licensure Or Eligibility For Licensure As A Social Worker In The State Of Pennsylvania.
