
Social Worker - Inpatient Rehab

We are currently seeking a compassionate and driven Social Worker to join our Inpatient Rehab team. In this role, you will play a critical part in helping children and their families navigate the challenges of rehabilitation and recovery. - Conduct assessments: Conduct comprehensive assessments of patients and their families to determine their psychosocial needs and develop individualized treatment plans.
- Provide counseling: Offer emotional support and counseling to patients and families to help them cope with the challenges of rehabilitation and recovery.
- Coordinate care: Collaborate with the interdisciplinary team to coordinate and integrate social work services into the overall treatment plan.
- Advocate for patients: Advocate for patients and their families to ensure their needs are met and their voices are heard in the healthcare system.
- Address barriers: Identify and address any barriers that may prevent patients and families from accessing necessary resources and support.
- Educate patients and families: Educate patients and families about available resources and support services to help them navigate the healthcare system and promote independence.
- Collaborate with community resources: Develop relationships with community organizations and resources to increase access to services for patients and families.
- Participate in discharge planning: Work with the team to develop and implement comprehensive discharge plans that address patients' physical, emotional, and social needs.
- Document and maintain records: Maintain accurate and timely documentation of patient care and progress, and ensure confidentiality of all patient information.
- Stay up-to-date on best practices: Keep abreast of the latest research and best practices in social work and integrate them into practice to provide high-quality care.
- Attend meetings and trainings: Attend team meetings, trainings, and conferences to stay informed and enhance professional development.
- Adhere to ethical standards: Adhere to ethical standards and guidelines set forth by professional organizations and the hospital.
- Collaborate with colleagues: Work collaboratively with other social workers and healthcare professionals to provide comprehensive care to patients and families.
- Provide crisis intervention: Respond to crisis situations and provide appropriate support and interventions to patients and families.
- Foster a positive work environment: Contribute to a positive and supportive work environment by promoting teamwork, respect, and open communication.
Master's Degree In Social Work From An Accredited Program.
Active State License To Practice As A Social Worker.
Minimum Of 2 Years Of Experience In An Inpatient Rehabilitation Setting.
Knowledge And Understanding Of Child Development, Family Dynamics, And Trauma-Informed Care.
Ability To Collaborate With Interdisciplinary Team Members And Provide Culturally Sensitive Care To Diverse Populations.
