We are looking for an experienced and compassionate Registered Nurse to join our Case Management team and provide exceptional care to our patients and families. As an RN Case Management Coordinator, you will have the opportunity to work with a team to ensure patients receive the best quality care and are supported through their healthcare journey. The ideal candidate for this role will have at least 5 years of professional nursing experience, a Bachelor’s Degree in Nursing, and a valid state nursing license. Previous experience in case management or discharge planning is preferred. The successful candidate should have excellent communication skills and the ability to manage multiple tasks simultaneously. Responsibilities: -Provide exceptional care and support to patients and their families throughout their healthcare journey -Assess patients’ needs and develop individualized care plans -Collaborate with other healthcare providers to ensure patients receive the highest quality of care -Monitor and evaluate patient progress and adjust care plans accordingly -Provide discharge planning and post-discharge support -Ensure compliance with regulatory requirements and healthcare policies -Maintain accurate patient records and documents -Provide patient and family education regarding healthcare needs and options -Participate in team meetings and other organizational initiatives -Maintain current knowledge of healthcare trends and best practices
