
Registered Nurse Inpatient Case Manager

- Coordinate and manage the care of inpatient pediatric patients and their families to ensure the highest quality of care is provided.
- Collaborate with multidisciplinary healthcare team to develop and implement individualized care plans for patients.
- Conduct comprehensive assessments of patients' medical, social, and environmental needs to identify potential barriers to care.
- Utilize clinical knowledge and critical thinking skills to identify and address potential patient care issues.
- Communicate effectively with patients, families, and healthcare team members to ensure continuity of care and patient satisfaction.
- Monitor patient progress and make appropriate interventions to ensure optimal outcomes.
- Collaboratively develop discharge plans and coordinate post-discharge care with community resources.
- Serve as an advocate for patients and families, ensuring their needs and preferences are considered in decision-making.
- Document all patient interactions and interventions in a timely and accurate manner.
- Participate in quality improvement initiatives to promote best practices and improve patient outcomes.
- Stay up-to-date with current healthcare practices and regulations to ensure compliance and provide the best possible care.

Bachelor's Degree In Nursing (Bsn) From An Accredited Institution.
Current Rn License In The State Of Pennsylvania.
Minimum Of 3 Years Of Clinical Experience In A Pediatric Inpatient Setting.
Certification In Case Management (Ccm) Preferred.
Strong Communication, Organizational, And Critical Thinking Skills.
