Job Description

At Children's Hospital of Philadelphia, we are looking for an experienced Pharmacy Care Coordinator who will be responsible for providing comprehensive pharmacy services to our pediatric patients. The ideal candidate should have a passion for delivering superior patient care and be committed to improving the health of children.The Pharmacy Care Coordinator will provide direct patient care services in the outpatient pharmacy setting, including medication therapy management, medication reconciliation, and medication education. The successful candidate should have a Doctor of Pharmacy degree as well as at least two years of experience in pediatric pharmacy practice, preferably in a hospital setting. The Pharmacy Care Coordinator should also have excellent interpersonal skills and be knowledgeable of applicable laws and regulations.This is an exciting opportunity for an experienced professional to join the pharmacy team at Children's Hospital of Philadelphia and make a positive impact on the lives of our pediatric patients.

Responsibilities:

  1. Provide direct patient care services in the outpatient pharmacy setting, including medication therapy management, medication reconciliation, and medication education.
  2. Develop and implement initiatives to improve the delivery of patient care and services.
  3. Ensure compliance with applicable laws and regulations.
  4. Maintain current knowledge of pediatric pharmacy practice and related topics.
  5. Participate in patient safety activities and initiatives.
  6. Create and maintain patient records and provide follow-up care as needed.
  7. Train and mentor pharmacy staff on topics related to pediatric pharmacy practice and patient care.
  8. Collaborate with other healthcare professionals to ensure the best care for patients.
  9. Evaluate and monitor patient progress and provide feedback to appropriate personnel.
  10. Provide excellent customer service to patients and families.
