Job Description

We are currently seeking a passionate and highly skilled Perinatal Social Worker to join our team and support our patients during their most vulnerable time - the perinatal period.   1. Provide emotional support and counseling to perinatal patients and their families during their hospital stay.
  2. Collaborate with the interdisciplinary team to develop individualized care plans for patients and their families.
  3. Conduct psychosocial assessments and provide ongoing support and interventions as needed.
  4. Educate patients and families on available resources and support services both within and outside of the hospital.
  5. Advocate for patients and families, ensuring their needs and concerns are addressed and integrated into the care plan.
  6. Facilitate communication and coordination between patients, families, and healthcare providers.
  7. Conduct follow-up visits and provide ongoing support to patients and families after discharge.
  8. Stay up-to-date on current research and best practices in perinatal care and integrate into daily practice.
  9. Maintain accurate and thorough records of patient interactions and progress.
  10. Participate in team meetings and provide input on patient care and program development.
  11. Collaborate with community organizations and agencies to provide additional support and resources for patients and families.
  12. Adhere to all hospital policies and procedures, as well as ethical and legal standards.
  13. Continuously evaluate and improve upon social work interventions and services for perinatal patients and families.
  14. Maintain confidentiality and respect the privacy and dignity of patients and families.
  15. Represent the hospital and social work profession in a professional and compassionate manner.

Job Qualifications
  • Master's Degree In Social Work From An Accredited Institution, With A Focus On Perinatal Care And Child Development.

  • Minimum Of 3 Years Of Experience Working As A Social Worker In A Healthcare Setting, Preferably With A Focus On Perinatal Care And Child Health.

  • Strong Knowledge Of Perinatal Care Processes And Protocols, Including Prenatal Care, Labor And Delivery, Postpartum Care, And Neonatal Care.

  • Experience Working With Diverse Populations, Including Low-Income Families, Refugees, And Families With Special Healthcare Needs.

  • Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Multidisciplinary Teams And Provide Support To Patients And Families During A Vulnerable Time.
