Job Description

The Children's Hospital of Philadelphia is looking for an experienced Patient Placement Coordinator who is passionate about providing exceptional care to children and families. This individual will play a key role in the hospital's patient placement process, ensuring that all patients are placed in the most appropriate care setting in a timely manner. As a Patient Placement Coordinator, you will be responsible for the timely placement of patients in the appropriate level of care, maintaining accurate patient records, and providing support to the patient's family throughout the placement process. The ideal candidate will have a strong background in healthcare administration, excellent organizational and communication skills, and a commitment to providing the highest quality of care to all patients. Responsibilities:

  1. Develop and implement a comprehensive patient placement process.
  2. Maintain accurate patient records and ensure that all patient information is up to date.
  3. Work with the patient’s family to ensure they are informed and provided with appropriate resources.
  4. Work with other healthcare professionals to identify the most appropriate care setting for each patient.
  5. Monitor patient placement process to ensure efficient and timely placement of patients.
  6. Develop and implement strategies to improve the patient placement process.
  7. Provide support to the patient’s family throughout the placement process.
  8. Stay up to date on relevant healthcare regulations and legislation.
  9. Troubleshoot any issues that arise with the patient placement process.
  10. Assist in the development of educational materials and resources for patients and families.
