- Serve as the primary point of contact for refugee families seeking care at Children's Hospital of Philadelphia, ensuring timely and efficient coordination of services.
- Conduct initial assessments and develop individualized care plans for refugee patients, taking into consideration their unique medical, social, and cultural needs.
- Collaborate with multidisciplinary teams to coordinate and facilitate necessary medical appointments, diagnostic tests, and procedures for refugee patients.
- Educate and advocate for refugee patients and their families, ensuring that they understand their treatment plans and have access to necessary resources and support.
- Lead cultural competency training for hospital staff to promote understanding and sensitivity towards refugee patients and their families.
- Monitor and track patient outcomes and identify areas for improvement in the delivery of care for refugee patients.
- Stay updated on current research and best practices related to refugee health, and incorporate this knowledge into the development and implementation of care plans.
- Maintain accurate and thorough documentation of patient interactions and progress.
- Collaborate with community organizations and agencies to ensure that refugee patients have access to necessary social services and support.
- Participate in program development and quality improvement initiatives to enhance the services provided to refugee patients.
- Serve as a liaison between the hospital and refugee communities, building trust and promoting collaboration.
- Maintain confidentiality and adhere to ethical standards in all interactions with refugee patients and families.
- Continuously evaluate and improve the Refugee Health Program and its services based on feedback from patients, families, and staff.
- Participate in professional development activities to enhance knowledge and skills related to refugee health.
- Embody the mission and values of Children's Hospital of Philadelphia in all interactions with patients, families, and colleagues.
Current Rn License In The State Of Pennsylvania
Bachelor's Degree In Nursing Or Related Field
Minimum Of 3 Years Of Clinical Experience Working With Refugee Populations
Knowledge Of Cultural Competency And Sensitivity In Working With Diverse Communities
Strong Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Interdisciplinary Teams And Community Partners.
