- Provide compassionate and high-quality nursing care to pediatric patients in their homes.
- Conduct initial assessments of patients' health and develop individualized care plans.
- Administer medications, treatments, and procedures as prescribed by physicians.
- Monitor and document patients' progress, including vital signs, symptoms, and behaviors.
- Educate patients and their families on health conditions, treatments, and self-care techniques.
- Collaborate with physicians and other healthcare professionals to ensure coordinated and effective care.
- Communicate with patients' primary care providers and provide updates on their progress.
- Assess and address any barriers to care, such as financial or cultural, that may impact the patient's treatment.
- Troubleshoot and manage any medical equipment or technology necessary for home care.
- Maintain accurate and up-to-date patient records and documentation.
- Ensure compliance with all healthcare regulations and standards.
- Participate in ongoing education and training to enhance knowledge and skills.
- Serve as an advocate for patients and their families, addressing their concerns and needs.
- Participate in interdisciplinary team meetings to discuss patient care and outcomes.
- Foster a welcoming, inclusive, and supportive environment for patients and their families.
Active Registered Nurse (Rn) License In The State Of Pennsylvania
Bachelor Of Science In Nursing (Bsn) Degree From An Accredited Institution
Minimum Of 2 Years Of Clinical Nursing Experience, Preferably In Pediatrics Or Home Care Setting
Certification In Pediatric Advanced Life Support (Pals) And Basic Life Support (Bls)
Strong Communication And Critical Thinking Skills, As Well As The Ability To Work Independently And As Part Of A Multidisciplinary Team.
