Diabetes Consult Nurse Practitioner

The successful candidate will work with an interdisciplinary team to provide comprehensive care coordination for our patients with type 1 and type 2 diabetes. Responsibilities:

1. Provide comprehensive care coordination to patients with type 1 and type 2 diabetes.

8. Participate in research and/or educational activities, if requested.
