
Diabetes Consult Nurse Practitioner
The Children's Hospital of Philadelphia is looking to hire an experienced Diabetes Consult Nurse Practitioner to join our team. The successful candidate will work with an interdisciplinary team to provide comprehensive care coordination for our patients with type 1 and type 2 diabetes. We are looking for a passionate, dedicated professional who is eager to join a vibrant and supportive environment.The ideal candidate for this role will have experience in providing diabetes care and a strong commitment to patient-centered care. Responsibilities:
- Provide comprehensive care coordination to patients with type 1 and type 2 diabetes.
- Develop patient-centered care plans tailored to individual needs.
- Utilize excellent communication and clinical documentation skills to provide high-quality care.
- Collaborate with an interdisciplinary team to ensure patient progress is tracked and documented.
- Maintain a current and valid Nurse Practitioner License in the state of Pennsylvania.
- Utilize experience and knowledge to provide the highest quality of care to patients.
- Take initiative to remain up to date on changes in the field of diabetes care.
- Participate in research and/or educational activities, if requested.
